These type of accidents are all to common, It really breaks my heart to hear of such a preventable accident. It really boils down to a lack of training and awareness of the risks associated with these machines. I always make sure to explain to candidates the correct travel position for these boom lifts and all types of MEWP'S and the standing position of their spotter when the machine is moving. Too many people have lost their lives due to the operator not having the machine in its correct travel position.

    Worker suffers life changing injuries that result in the amputation of their lower leg, whilst the company is fined £400K....... both of which were completely avoidable.The company failed to have a suitable and sufficient Safe System of Work in place.     Several key points stand out from the Health and Safety Executive HSE's findings:1. Risk Assessment and Safe Systems of Work: The failure to implement a suitable and sufficient safe system of work for the movement of the MEWP was a fundamental lapse in health and safety management. It is vital that all activities involving heavy machinery are underpinned by thorough risk assessments that lead to the establishment of robust safe systems of work. These systems must be clearly communicated, understood, and consistently applied.2. Training and Information: The lack of adequate training and information regarding the movement of the MEWP and the role of a banksman is deeply concerning. As health and safety professionals, we must ensure that all employees receive appropriate training that is specific to their tasks and responsibilities, particularly when they involve complex or hazardous operations.3. Policy and Practice Alignment: It is not enough for a company to have safety policies in place if they are not adhered to in practice. The disconnect between Baker & Baker Products' normal practice and company policy indicates a breakdown in the implementation and monitoring of safety procedures.4. Vehicle and Pedestrian Segregation: The principle of keeping people and vehicles apart is a cornerstone of vehicle safety in the workplace. This incident underscores the need for clear separation and the implementation of control measures such as designated walkways, barriers, and effective supervision.5. Communication: Effective communication between the banksman and the MEWP operator is essential to prevent accidents. The lack of appropriate banksman training, including communication protocols, was a critical oversight.My recommendations in the wake of this incident would include:- Comprehensive Review: Conduct a thorough review of all current risk assessments and safe systems of work, with a particular focus on vehicle operations.- Training Audit: Undertake an audit of training records to ensure all staff are adequately trained for their roles and that refresher training is scheduled appropriately.- Policy Enforcement: Reinforce the importance of adhering to company safety policies, with clear consequences for non-compliance.- Culture of Safety: Work towards fostering a culture of safety where all employees feel responsible for not only their own safety but that of their colleagues, encouraging reporting of unsafe practices.- Continuous Improvement: Implement a continuous improvement approach to health and safety, ensuring that lessons learned from incidents are integrated into future safety planning and practice.The long-term impact on Mrs. Bramhall's life is a stark reminder that behind every health and safety statistic is a person whose life may be forever changed by workplace accidents. It is our duty to uphold the highest standards of health and safety to prevent such devastating events from occurring.
